About Us

About the Role
As a Support Coordinator, you will play a vital role in helping our participants break through barriers and actively participate in their local communities.
Key responsibilities:
- Implementing, connecting, and coordinating supports.
- Making effective referrals to preferred services.
- Providing guidance for NDIS plan reviews and change of circumstances.
- Supporting participants during crisis and complex situations.
- Collaborating with various mainstream and community services.
- Helping participants build capacity and develop meaningful relationships.
About You
Qualifications, skills, and experience:
- Extensive experience in NDIS Support Coordination / Case Management (3–10+ years).
- Tertiary qualification in Allied Health, Social Work, Counselling, Nursing, or Human Services (or significant NDIS Support Coordination experience).
- Proven ability to work both independently and collaboratively in a team.
- Strong rapport-building skills with diverse customers.
- Expertise in supporting customers with complex needs.
- Energetic, driven, and committed.
- Excellent communication, administrative, and documentation skills, including proficiency with online portals and databases.
- Established connections with disability, mainstream, and community networks.
- Friendly and positive personality, open and adaptable to innovation.
- Valid driver’s licence (essential).
- Mental health expertise (desirable).
- Experience with acquired brain injury or spinal injury (desirable).
- Current Working with Children Check and Police Check (essential or willing to obtain).
- NDIS Worker Screening and Orientation (essential or willing to obtain).
